Please read the following information carefully prior to downloading a copy of the application:

• Enrollment is based on available funding. Submitting an application DOES NOT guarantee enrollment into the program.
   

• Applicants are enrolled in the priority order by site category as established by the Florida Legislature in Section 1009.66, Florida Statutes. For a list of the sites in priority order, please visit the Designated Sites section of this website.
   

• It is the applicant's responsibility to ensure that the application packet is completed before being submitted to the program. A complete packet consists of the following pages with appropriate signatures/stamps and all fields completed: Initial Application (Page 6), Employment Verification (Page 7), Loan Principal Certification (Pages 8 & 9), Copies of Nursing Diploma and License. Applications are not reviewed until after the deadline.
   

• Applications are ONLY accepted between:

• Participants are responsible for maintaining their scheduled payments throughout their participation in the program. Enrollment in the NSLFP DOES NOT exclude you from monthly payments.
